There are basically 4 types of fault
Symmetrical fault is
1. 3phase fault and
Unsymmetrical faults are
2.Line to ground fault,
3.line to line fault and
4.line to line to ground fault.
Phase fault occurs when one of the line falls on any ground structure.
line to line fault occurs when any two lines of the 3 come in contact with each other without any physical contact with the ground...
Phase to phase fault occurs when any two lines of the 3 come in contact with the ground structure, in other words phase to phase fault is line to line to ground fault.

Phase to Phase fault.
when you have 3 ph supply, at that time if any one phase came in contact with another phase without any Ground Contac and draw effectively high current and trip the circuit. that is called Phase to Phase fault. in this condition your connected load may be affected or damage due to failure of 2 phases.

I have 2500KVA transformer delta/star,13.8KV/380V provided with LV 4000A breaker. Already I have 2300KW motor feeder with a full load 90%. Is it possible to connect another 1600A from that breaker? If so how the calculation is done?

We all have seen transmission and Distribution lines In transmission lines the voltage is high and current is low(to reduce I^2r loss) and in distribution lines voltage is low but current is high however the size of transmission wires is bigger than that of distribution wires even though the amount of current carried by the distribution lines is more than that of transmission lines why?
