Incomer side or outgoing side current transformer terminals connected wrongly by mistake.so reverse current will flow in the secondary.secondary is connected to the metering device and protective devices.so protective devices will sense the reverse current flow.if that current reaches certain level,this protective device trip the circuit breaker.
If both side CB's are tripping simultaneously means transformer winding side only fault happened.
