No where we face this situation where two people are
working on same module. Before starting testing only we
have a test plan on who to work on what. If their is a
situation where two people need to interact with teh same
module then it is the place where we need co-ordination.

If u r familiar with the quality center HP tools, in defects table there is a one icon called find similar defect when u click after writing the defect it pop up the message that some one already log same defect. so u follow next procedure.
