That depends on the model which organization is following....
If it is following V-Model, then QA team is necessary to
verify the documents are designed as per the requirements...

attend the discussions with client in order to keep track

yes , QA involves Entire on Software Development process.
the whole design process is improved and monitored
by QA and also to find out any agreed upon standards are
followed or not.
