I answered
1) After i found a bug i will reproduce that bug at my site
2 to 3 times. I will capture all the screen-shots needed to
provide as a evidance.
2) I will prepare a detailed defect report(Tool or Excel),
with all parameters like detailed bug description, steps to
reproduce, severity, priority, type of defect, build No,
environment, screenshots, mention any special condition.
3) And report that bug to the devloper or concerned person
and keep on following with the same.

The things which we should follow before raising a defect;
1. First analyze whether its really a defect
2. Check its reproducibility on Build which is under test
3. Check its reproduciblity on previous build so that we can
get info like defect is regression or new defect.
4. Check whether it needs particular environment for its
reproducibility
5. Decide its severity based on impact on application
6.Then we should report it in a bug tracking tool
