as far as I know big engines starts with external motors
which is called self motor and they are operated with
batterys and once the engine is started the battery is
charged with alternater driven by the same engine connected
with belt

in heavy vehicles like cars the flywheel is first rotated
with help of external aid like motors due to which the
piston gains momentum and the engine start operating on the
specified cycle.

compressed air. compressed ait is used to move the poston
of the engine.

yes, piston movement is required to start the engine. for
vehicle , gear remains disengaged that's why vehicle is not
moving forward.
