THE MAIN POINT IS DUE TO EXCESSIVE HEAT, THE REASON MAY BE AS LOSS CONNECTION,CONTAMINATION OF TRF. OIL WITH MOISTURE,WITH NOT PROPER COOLING, AND EXCESSIVE LOAD.

1:use of of improper conductor on termination
2:oveheating of transformer either due excessively load or failure of cooling system or internal fault.
3:loose connection on bush
