Risks are documented in Project plan and daily risks related
to releases are documented in Weekly Status reports and the
tools we use for tracking the project activities.Each
company have their own tools.

Also project specific risk can be documented in Risk Log
which will contain the following

1. Risk Description
2. probabiltiy of occrence
3. impact
4. prority
5. Mitigation Plan
6. Contengency plan

These risk can be identifed during the initial stages of
theproject (norming session)

It can be discussed in WPR report
