PA stands for Preventive action ( the potential non-
conformities?)
root cause analysis is done after the Internal audit. when
any non conformities arises, root cause analysis is done
for the same. root cause can be of any form : client
miscommunication, programming errors, misunderstanding, not
clear requirement
