Its depends on the comany and the project.
Suppose the requirements are keep on changing or the client
doesnt have the clear idea about what exactly he wants then
we can go for PROTOTYPE MODEL.
Most of the standard oprganizations go for V model.

In any application there are so many fields. So tester have to test each field with Special chars., Alphabetic value, Alphanumeric value, Blank Space. etc. I am testing with all these inputs but should I have to consider these into test cases.
