SDLC is large and complex process and it has human interaction. so error or mistake is very simple issue. It could be initial phase when requirement gathering, it could be have defects in design phase, it could be developing phase. so u have defects and u r going to delivery your product to the clients, its make sense, we all agree thats not a good idea. So bring QA guy who find out the defects and u fix it , then u go to production. u happy, customer happy your goodwill increase, your business will be grow.
