why in a transformer frequency remains constant? if frequency
is changing what will happen?

As the secondary voltage is induced by the primary current
the frequency will follow each other. Normally the
frequency of the supply is constant so the output frequency
will remain the same.
If the supply frequency changes the output frequency will
follow this change and change also.
