md: dxx: write error on /dev/dsk/c*t*d*s*

when u check metastat -c. Everything will show clean. we
need to replace the disk in the server.

Issue 2 : metastat -c will show both the disk in
maintenence.
- u need to run the metasync -r . issue will be cleared.

Issue 3 : Usually disk slice in maintennce.
-- Need to check the iostat -en, check the disk,maintence
disk has hard error . if no attach the disk and check. if
found hard errors are increasinG or more than 3. - replace
the disk.
