If the report is prepared manually from the defects raised
then there is a possibility of error in the report. The
best way to pull the defect report. For this try to follow
the test management tool like Test director or tracking
tools like bugzilla to log the defects.
Once after pulling the report do a review by having check
points.
This will ensure the report with high accuracy
