What is the formula for the calculation of Gratuity?

please calculate the gratuity payable for an employee who
was appointed on July 1999 to 13.04.2000 as adhoc employee
and regulirised w.e.f 14.04.2000 and served this
organisation upto 28.11.2008. The basic pay as on nov 2008
is Rs. 5375/ and DA Rs. 2822/pm.
please show me the caculation
