Accident/Incident Report should have a Preliminary &
followup Report. Preliminary Report contains details like
Name of the victim, equipment or an property damage & their
details and Place of Happening, date & time of incident.
Immediate Supervisor or in charge name contact no., Brief
description of the incident happened, Name of any witness.
Check Whether Safety precautions done as per Legal, Company
policy, procedures are followed are not and identify the
causes if required. Check whther Risk Assessment is done
for that activity. All the above points should be
considered in making the report.

Followup report should contain the description of the
accident to be filled by the in charge or supervisor after
investigating the incident and to be reported within say 3
days.. This also caontains the Senior or Managers comments
to avoid the incident.
Followup report will be replied by HSE or higher authority
with recommendations to avoid such incidents...so a
corrective action sheet is also included along with
followup report which will have recommendations from HSe or
higher authority and Action taken for the recommendation
which has to be replied by Manager of the concern or victim.

All officials involved like supervisor, manager, HSE
personnel should sign at respective places in the report.

A time frame to be set for replying for the recommendations
by HSE.

We are preparing another project which involves Waste heat recovery from a 1 MW GE Engine. The Engine in particular gives out flue gas at 487Deg C with a flow rate of 5650 kg/hour and this can be cooled to 180 Deg C. This gives a thermal output of ~ 594 KW that is available for input to VAM via heat exchanger / boiler. Additionally 581 KW will be available from engine jacket water (temperature profile 80/ 80 Deg C and a flow rate of 50m3/hr) Our estimation is that 300 TR of cooling is possible with a suitable VAM machine(s). Please advise what is the most suitable arrangement with maximum TR and min Capital expenditure. plz reply me as early as possible
