First A bug can be reported by using a Bug tracking tools
(like Quality Center,Test Director, Bugs,Bugzilla, through
microsoft Outlook/IBM Lotus notes).A bug should be reported
by using the reproducible steps (the steps by which the bug
can be reproduceded).Screenshot/Attachments of the bug
arasied application. Assign to the developer or System
Analyst or Project Manager( who then assign to the
corresponding Developer).
There are different hirarchies to assign an issue/bug
depending upon the Organization or Campany
Tester <---->PM <--->System Analyst <---> developer
Tester <---> System Analyst <--> developer
