I don't know the process in your company, which methodology
you follow - but the right thing to do is address the gap
to the document "author" in which document you find the
gap and request to update the document.
The main thing here is the "GAP" (which could be a missing
requirement, incorrect req, conflicting with other req)
should be addressed by the identified person only. since it
makes a lot of difference if you go through a layer process
to address it instead of addressing directly.
