He will report to the Project Manager. Project Manager will
arrange a meeting with all the leads (Dev. Manager, Test
Lead and Requirement Manager) then check impact of defect ,
Then development team gives hot fixed of defect & testing team retest this defect
Then that part will deploy to client in live environment
