Both Are the Same but servers different purposes.
Report Service Document: is a detailed version of Report data which include widgets and graph to anylsis the data throughly. You can use every section of the document body e.g. Page Header/Footer, Document Header/Footer Layout Header/Footer Details Sections and Group Header/Footer if it is in design.
Report Service Dashboard: is more summerise and brief version of a document.
Dashboard are meant to be more interactive, inquitive.
You mainly use Detail Header section to create a dashboard out of document.
Several Widgets DHML/Flash/Mobile servers the purpose of interactivity and more sophisticated BI solution.
