tell me some req'r which were added to frs , in the middle
of the project?

Whenever there is a change in the fr's in the middle of the
project, it is due to the changes requirement of the
customer which he feels needs to be altered as he has
reviewed the project in the initial or first phase of
development and finds out something to be incorrect or
requires additional functionality.
This could be either addition of some new requirements or
the extension of the existing.
Lets say i have if i have a login page which has Username,
password and Ok button, but in between the project
development the customer feels a need of the Cancel button
which he decides in between the development phase.

You must test a trouble report where the problem cannot be reproduced. You tried using various test data files and mutations from the data files the customer sent along with the trouble report. The software does not fail the way it did at the customer's site.
