There is no such thing as defect-free software. If there is
then it only remains defect-free for a short time as there
are always changes to requirements that cause software to
be changed and require retesting.
IMO the sign of a system that is close to being tested well
enough, is based on the type of defects being raised. The
severity is lower and the defects are more cosmetic in
nature.

Scenario A year ago we developed a standards compliant website for a client. Their site is very popular resulting in the client wanting to be able to track how it is being used by their users. The first part of this project is to create a Login function so that users can be identified on the website. The data supplied by users would have to be handled securely.
