Cause: The specified plan table does cannot be found.
Action: Create the specified plan table or use an existing
plan table.

1. run toadprep.sql
2. go to VIEW menu, click OPTIONS, click ORACLE in the tree
view, then change EXPLAIN PLAN TABLE NAME into PLAN_TABLE
