this is a condition which is known as methemoglobinemia.it is caused due to ingestion of large amounts of nitrates in water by an infant. these nitrates are converted into nitrite by the digestive system.The nitrite then reacts with oxyhemoglobin to form metheglobin, consequently the body tissues may be deprived of oxygen causing the infant to develop a blue coloration of their mucous membranes.

It is usually caused by a heart defect which laymen often called "a hole in the heart". Normally, oxygenated blood from the lungs is separated from deoxygenated blood from other tissues. A defect in the heart ventricle walls can allow deoxygenated blood to mix with the oxygenated blood . The resulting blood going through the aorta has less oxygen than usual, and the baby looks blue.
